When conducting an assessment, you may want to consider an informed consent form for participants to agree to. Typically projects going through the IRB process will need to include an informed consent process, so the attached form could be helpful in your IRB protocol. The informed consent text usually lets participants know the purpose of your study, terms of confidentiality, and provides contact information if a participant has questions. 

If you are administering an online survey, you can include the informed consent in the email text stating that they are consenting to participate by clicking on the link. Conversely, you can provide the informed consent on the first page of the study and indicate participants consent when they click "Next" to begin the survey. With paper surveys, focus groups, interviews, etc., you may have participants sign a paper form indicating their consent.
